Why Your Shoulder Hurts Most at Night — And What Helps
You've had a fine day. You've been at your desk, been to the gym, maybe even forgotten about your shoulder entirely — and then you lie down to sleep and it starts throbbing. For a lot of people with shoulder pain, night-time is when it's worst, and it's one of the most common complaints we hear at the clinic. It's also one of the most frustrating, because rest is supposed to help, not hurt.
There's a real physiological reason for this, and understanding it makes the fix much more logical.
Why lying down makes it worse
During the day, gravity is broadly on your side. Your arm hangs down, blood and fluid drain away from the shoulder joint, and movement keeps things mobile. At night, several things change at once. Lying flat reduces venous and lymphatic drainage from the arm, so fluid and inflammatory mediators pool around the joint rather than being cleared. Your body's natural cortisol levels — which have a mild anti-inflammatory effect — drop overnight, so inflamed tissue is less "damped down" than it is during the day. And whichever way you're lying, you're almost certainly compressing or stretching the structures that are already irritated, whether that's a rotator cuff tendon caught under the acromion or a stiff, inflamed joint capsule.
Put simply: the shoulder doesn't get worse overnight because you've done more damage. It gets worse because the conditions that usually keep swelling and irritation in check are switched off.
The usual suspects
Two conditions account for most night-dominant shoulder pain we see.
Rotator cuff tendinopathy or impingement is the most common. The tendons that stabilise and move the shoulder can become irritated from overuse, poor movement patterns, or simply age-related wear. Lying on the affected side compresses the tendon directly; lying on your back can still narrow the space it runs through, especially if you sleep with your arm overhead. This is often the pattern in gym-goers doing a lot of pressing or overhead work, and in people who've had a sudden increase in activity.
Frozen shoulder (adhesive capsulitis) tends to cause a deeper, more diffuse ache that's notoriously worse at night, often disturbing sleep for weeks or months at a time, alongside a progressive loss of movement in multiple directions. It's more common in people in their 40s and 50s, and in people with diabetes.
Less commonly, night pain can come from referred issues in the neck, or from arthritic changes in the joint itself. Because the underlying cause changes the treatment completely, this is one of the areas where a proper assessment — rather than guesswork — makes the biggest difference. Sleep positions and pillow strategies
While you're working on the underlying cause, a few practical changes can genuinely reduce night symptoms:
- Avoid sleeping directly on the affected shoulder. This is the single biggest culprit for compressive pain. If you're a natural side-sleeper, try the other side with a pillow hugged to your chest to stop you rolling back.
- Support the arm if you're on your back. A small pillow or rolled towel under the upper arm keeps it slightly elevated and prevents it dropping into a stretched, provocative position.
- Keep the arm below shoulder height. Sleeping with your hand under the pillow or arm overhead narrows the subacromial space and tends to wind up impingement pain.
- A slightly propped position (using an extra pillow to sleep semi-reclined) can help in the first few nights of a bad flare, as it reduces the compressive load through the joint.
None of these fix the underlying problem, but they buy you sleep — and better sleep genuinely improves pain sensitivity and recovery.
Daytime habits that make a difference
What you do in daylight hours has more influence on your nights than most people expect. Gentle, regular movement through the shoulder's available range — rather than complete rest — helps keep fluid moving and the joint from stiffening further. Conversely, overloading it with heavy overhead pressing, lifting, or repetitive throwing-type movements during the day tends to show up as pain that night, sometimes with a delay.

When to get it looked at
Occasional night discomfort after a heavy gym session is normal. But shoulder pain that's consistently waking you, lasting more than two to three weeks, or accompanied by significant loss of movement (struggling to reach behind your back or overhead) is worth having assessed properly rather than waiting it out. Frozen shoulder in particular tends to progress through distinct phases, and starting the right management early can shorten how long it takes to settle.
