    Sciatica vs General Back Pain: How to Tell the Difference

    Back pain is one of the most common reasons people come to see us, but "back pain" isn't really one thing. Some patients describe a dull, tight ache across the lower back that flares up after sitting or gardening. Others describe something quite different — a sharp, burning pain that shoots down one leg, sometimes with pins and needles or numbness in the foot. These are two different problems with two different treatment approaches, and telling them apart early makes a real difference to how quickly you recover.

    What sciatica actually is

    Sciatica isn't a diagnosis in itself — it's a description of what happens when the sciatic nerve, or one of the nerve roots that feeds into it, gets irritated or compressed as it exits the spine. This is most often caused by a disc bulge pressing on a nerve root in the lower back, though it can also happen due to degenerative changes or, less commonly, muscle tightness compressing the nerve as it runs through the buttock.

    Because the sciatic nerve runs from your lower back, through your buttock, down the back of your thigh and into your calf and foot, irritation to it tends to cause symptoms well beyond the back itself. That's the biggest clue.

    The key differences to look for

    Where you feel it. General mechanical back pain tends to stay local — across the lower back, sometimes into the buttocks, but rarely much further. Sciatica characteristically travels: pain that runs down one leg, often below the knee, following a fairly consistent path rather than jumping around.

    What it feels like. Mechanical back pain is usually described as an ache, a stiffness, or a spasm-like tightness. Sciatica often has a different quality — burning, shooting, or electric — and is frequently accompanied by pins and needles, numbness, or a sense of weakness in the leg or foot that pure back pain doesn't cause.

    What makes it worse. Both types can be aggravated by prolonged sitting, but sciatica is often distinctly worse with sitting, coughing, sneezing or straining, because these all increase pressure around the irritated nerve. Bending forward can also aggravate sciatic symptoms, whereas general back pain is often just as irritated by extending or twisting.

    One side or both. Sciatica is almost always one-sided. If you've got pain, tingling or weakness running down both legs at once, that's a different picture and worth having assessed promptly, as it can occasionally suggest more significant nerve involvement.

    Red flags — when to get seen urgently

    Most back and leg pain, sciatica included, settles with time and the right management. But there are a small number of warning signs that mean you shouldn't wait for a routine appointment and should seek same-day medical attention: numbness around the saddle area (inner thighs, groin, or around the back passage), new difficulty controlling your bladder or bowels, or rapidly worsening weakness in both legs. These can indicate cauda equina syndrome, a rare but serious condition requiring emergency care. It's uncommon, but it's important you know the signs.

    What actually helps

    For general mechanical back pain, the evidence is consistently reassuring: staying as active as symptoms allow, avoiding prolonged bed rest, and gradually reintroducing movement tends to work better than rest alone. A short course of guided physiotherapy can help identify what's driving the pain — often a combination of stiffness, muscle guarding, and movement patterns that have become protective rather than helpful — and get you moving confidently again.

    Sciatica generally needs a slightly different approach. In the early, more irritable phase, finding positions that ease nerve pressure (often lying down with knees supported, or standing rather than sitting for long periods) can reduce symptoms while things calm down. As irritability settles, targeted exercises to restore movement in the lower back and mobility in the nerve itself become important, alongside strengthening the muscles that support the spine so you're less likely to have a recurrence. Most people with sciatica improve within a matter of weeks without surgery, though it can take longer than typical back pain to fully settle.

Once the acute pain is under control, building strength and resilience through a structured strength and conditioning programme helps reduce the chance of it coming back, particularly important if your job involves a lot of sitting or your sport involves repeated bending and lifting.

    When to come and see us

    If your back pain is settling on its own within a week or two, that's entirely normal and doesn't necessarily need input from a physio. But if you're noticing pain, numbness or weakness travelling down a leg, if symptoms aren't easing after a couple of weeks, or if you're simply unsure which type of pain you're dealing with, it's worth getting it properly assessed rather than guessing. A clear diagnosis early on usually means a shorter, more straightforward road to recovery.
